Policy Review and Acceptance
There are several key policies that need to be reviewed and accepted digitally. These policies are found in myDSD.
Instructions:
- Log on to myDSD. This will take you to the "Student Information System."
- Click on the "Tools" tab on the top of the page and select "Sign Documents" from the drop-down menu.
- Review each policy and click the green "Accept" button.

Each State educational agency shall ensure that each child of a homeless individual and each homeless youth has equal access to the same free, appropriate public education, as provided to other children and youths. Davis School District has the responsibility to inform any homeless student of the services they are eligible to receive. NOTE: If you answer NO to the first question, please skip all other questions and submit the form.

Device Protection Plan
Students are responsible for the care of the device and will be issued a fee if device is damaged.
LHS is providing an OPTIONAL protection plan for damage to devices. If not enrolled in, parents/students carry full liability for the damaged laptop.
Plan Costs:
Cost: $25 per school year (nonrefundable and not prorated)
Fee Limit: Up to $250 for non-touch screen and $400 for touch screen (cost of Chromebooks)
Coverage: Repair or replacement of school issued laptop.
Students have until Monday, August 28th to purchase the device protection plan. New students, registering after August 28th, will have 2 weeks from their enrollment date to purchase the plan.

Yearbook Pictures / ID Badges
Pictures will be taken in front of the auditorium.
Thursday, August 17th : During English classes, Before (7am) and After school (3pm)
Friday, August 18th: During English classes, Before (7am) and After school (3pm)
Make Up Day Wednesday, September 20th: Before school (7am), during lunch and after school (3pm)
Seniors will be photographed in a graduation gown.
Students will be provided with their ID badge and a school lanyard on their picture day. Students are required to wear their ID badge in a visible way every day. This allows school personnel to ensure the safety of our school grounds. Students who forget their ID badge will be expected to obtain a disposable ID badge from the main office.

Parking Permit
- Parking permits may be purchased by all juniors and seniors. Due to limited parking availability, sophomore students will not be issued a parking permit (no exceptions).
- All vehicles parked on school property must be registered with the school and MUST ALWAYS DISPLAY a current permit.
- Cost of the parking permit is $5.00 per vehicle per semester and $5.00 for a replacement permit.
- Students will not be allowed to purchase a parking permit at the beginning of the school year or the beginning of the second semester if they have any outstanding U’s, or outstanding fees/ fines.
- New parking permits will be sold at the beginning of the second semester; all first semester U’s and or fines must be cleared and paid for before the new second semester passes will be issued.
- Students must not park in the following restricted areas: faculty, visitor, fire lane, handicap, etc. A $25 fine will be assessed for parking in handicap and/or fire lanes.
- Each parking violation will result in a $15 fine.
- Vehicles without a parking permit may be booted at any time/a fine may be given.
- After 3 parking citations, violating students may have their vehicle booted (boot fine is an additional $20).
- Repeat offenders may have their vehicle towed at owner expense.
- Parking privileges may be revoked at any time.
- Student parking and the purchase of a permit is on a first come first serve basis. The purchase of a parking permit does not guarantee a permit or a parking place each day.
- Any student that duplicates a parking pass, sells a parking pass to another student, steals a parking pass, or is in possession of a stolen parking pass will lose the privilege of parking on campus for the remainder of the school year, could be suspended from school, may be fined $25, receive an administrative “U”, and could be charged with theft.

Yearbooks
Yearbooks can be purchased through myDSD or in the Main Office. The cost is $50.
After October 31 the price goes to $55.
Many students choose to wait until the end of the year to purchase a yearbook. Be aware that we regularly sell out of yearbooks a few weeks prior to yearbook distribution and many students are unable to purchase one.

Child Find Notice
Davis School District is responsible for Child Find indentification and evaluation for all students suspected of having a disability residing in Davis County. If your child is having significant difficulty with vision, hearing, speech, behavior, is experiencing slow development typical for his/her age, physical development or learning difficulty, he/she may be a child with a disability that requires specialized instruction through special education. If you suspect that your school age child may have a disability, please contact the school Special Education rep or the special education department.
